1. Fees:  Fees for CranioSacral Therapy or Massage Therapy are:

60 minutes      $115.10 + HST  = $130.00

2. Hours of operation: I am available for treatments on Saturday and Sunday. Please contact me to set up an appointment.

3. Massage assessment:   For Massage Therapy a pre-assessment lasting 20 -30 minutes will be performed to evaluate the condition and establish an appropriate treatment plan. This will be negotiated prior to the first Massage Therapy session.

4. Clients running late: Please make every effort to be on time. If you are late you will forfeit that time for your session.

5. Therapist running late:  I will make every effort to be on time. If I am running late I will still provide you the full time of your treatment.

6. Cancellation: A minimum of 24 hour notice is required for the cancellation of your appointment to avoid late payment.  You will be responsible for the full fee of the therapy if you cancel the appointment within less than 24 hours.

7. Missed appointments:  Missed appointments will be charged for in full.

8. Confidentiality: All information in the therapy sessions will be held in confidence and only released to other practitioners with your consent. The College of Massage Therapists of Ontario has legal right to view client files.

9. Referral:  I reserve the right to refer to you another therapist in the event that your condition requires treatment beyond my scope of practice.
